About The Position

Fairview Health Services is seeking a Nurse Practitioner to join our established primary care practice at M Health Fairview Clinics – Oakdale in Oakdale, Minnesota. This is an excellent opportunity to join a collaborative, multi-specialty clinic. Practice details Established primary care clinic with 11 provider. Average patient volume of approximately 16–24 patients per day Opening: 1.0 FTE (34 patient contact hours) - flexible schedule. Fully integrated electronic medical record (Epic) Oakdale, Minnesota is a welcoming suburban community located just east of St. Paul, offering a small-town feel with easy access to the Twin Cities. Requirements

  • Graduate of an accredited ACGME/AOA Internal Medicine residency training program
  • Licensed or ability to obtain Minnesota Board of Medical Practice Licensure Upon Hire
  • BC/BE with the ABMS or AOA Upon Hire
  • Ability to obtain and /or maintain DEA certification for the State of Minnesota Upon Hire
  • Meet threshold criteria/qualifications for Credentialing and Privileges Upon Hire

Responsibilities

  • Examines patients, formulates differential diagnostic plans, defines and orders required diagnostic testing. Provides preventative, chronic disease, and acute care depending on the need of the clinic site. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery.
  • Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene.
  • Interprets examination findings and test results and implements treatment plans. Refers patient to medical specialist or another practitioner when necessary.
  • Completes medical record and electronic order management (via in basket); and completes cross coverage depending on site needs. Follows organization policies and procedures and protocols.
  • Actively participates in multidisciplinary team approach to case management
  • Participates in utilization and quality tools and activities that promote continuous improvement in delivery of patient care, patient outcomes or workflows.
  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedaures, and standards.
  • Actively participates in creating and implementing improvements.
  • Performs other responsibilities as needed/assigned
