About The Position

The Primary Care Coordinator is responsible for assisting in the management of all aspects of the patient intake process including communicating directly with patients and families, data entry, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, facilitating a daily intake stand up meeting, and managing the insurance verification and authorization processes.

Requirements

  • At least 18 years of age
  • Ability to read and follow written instructions and document care given
  • Satisfactory references from previous employers
  • Is self-directing with the ability to work with little direct supervision
  • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order
  • Good oral and written communications
  • Good organizational skills

Responsibilities

  • Assist providers with scheduling, record keeping, organization.
  • Manage Chronic Care Management charts.
  • Send orders and referrals for Provider.
  • Communicate and clearly explain services provided.
  • Answer phones and direct as needed
  • Audit medical records to ensure visits to are being made, notes are in the medical records, and that the notes are detailed
  • Coordinate referrals and following up with patient in a timely manner to coordinate admission
  • Assist with scanning
  • Assure orders are sent to physicians in a timely manner
  • Handling incoming calls and other communications
  • Maintain organization of the office common area
  • Obtains and documents prior authorization for hospice services from insurance providers
  • Assist with hospice and home health as needed.
  • Processing referrals, coordinating DME, obtaining medical records, orders and scheduling evaluations.
  • Assists to maintain liaison relationships with hospitals, facilities, insurance personnel, providing information and education on organization services, coverage issues and related areas.
  • Resolve appointment conflicts for staff
  • Assure death visits are available for RNs at time of death
  • Produce and process schedules and reports as they are requested by administrative staff
  • Assure admit packets are stocked and ready for both hospice, home health and palliative care admits
  • Assist with claim denials
