About The Position

Chaparral Medical Group is seeking a Spanish-speaking Advanced Practice Provider (APP) — Physician Assistant (PA) or Nurse Practitioner (NP) — to join our collaborative and mission-driven care team. Based in our primary care clinics across the Pomona Valley region, you will play a key role in delivering high-quality, compassionate care to a diverse patient population, including a large Spanish-speaking community. You will work alongside a collaborative team of clinicians, supported by integrated virtual care and operational support systems that allow you to focus on patient care and clinical excellence.

Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current national certification (NP or PA)
  • Active California license (or eligible for licensure prior to hire)
  • Fluent in Spanish and English (required)
  • Experience as an APP in outpatient primary care or internal medicine
  • Strong clinical judgment and diagnostic skills
  • Excellent communication and patient education abilities
  • Comfortable working in a team-based, value-driven care model

Nice To Haves

  • Experience serving Medi-Cal or value-based populations
  • Experience in community or underserved care settings

Responsibilities

  • Manage a patient panel and provide comprehensive primary care, including acute, chronic, and preventive care visits
  • Deliver in-person care at clinic sites, with occasional telehealth visits as appropriate
  • Provide care to a diverse patient population
  • Communicate clearly with patients from varied cultural and linguistic backgrounds, ensuring understanding of diagnoses, treatment plans, and preventive care
  • Collaborate with physicians, APPs, and clinical staff through daily huddles and care team coordination
  • Perform in-office procedures within the scope of your training and credentials
  • Coordinate care with specialists and community partners, ensuring smooth referrals and follow-ups
  • Maintain accurate and timely documentation in the electronic medical record system
  • Contribute to a culture of teamwork, quality improvement, and patient-centered care for the communities we serve
