We are seeking a float full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our Conviva Senior Primary Care team in beautiful, sunny Saint Lucie, Florida. With over 340 centers across 15 states operating under the CenterWell and Conviva brands, we're transforming healthcare by putting seniors at the heart of everything we do. As a Float Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. You will report out of our East St. Lucie clinic, and report to our Associate Medical Director. You will help cover clinics in Stewart, West St. Lucie, East St. Lucie, Central St. Lucie, Fort Pierce, and Lake Okeechobee. Requirements

  • Master's Degree in Nursing or completion of a PA program with board certification.
  • Active, unrestricted NP or PA license in the state of practice.

Nice To Haves

  • Experience in value-based care, primary care or Medicare Advantage settings.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

  • You will provide comprehensive care to seniors following clinical standards.
  • You will collaborate daily with your care team to ensure coordinated, high-quality care.
  • You will manage referrals, hospital/SNF coordination, DME, and home health services.
  • You will document care accurately and efficiently with support from quality-based coders.
  • You will participate in on-call rotation and support growth plans at your center.
  • You will have passion for providing value-based care.
