The Specialist, Prevention Navigator provides intensive care coordination, support, and services to people who inject drugs, many of whom are already clients of Vivent Health’s LifePoint Program, which provides syringe access services. The goals of Prevention Navigation are to help clients reduce barriers and build the skills needed to access services such as medical insurance, hepatitis C care, and addiction treatment services for those who are ready. The Specialist, Prevention Navigator will implement a client-centered approach rooted in harm reduction. This position will work with clients to develop an individualized plan to access prevention and treatment services for a broad range of health needs identified by the client. In addition to providing service coordination, the Specialist, Prevention Navigator will be instrumental in identifying, building, and expanding existing relationships with relevant community-based organizations and health centers to create a regional prevention referral network. The Specialist, Prevention Navigator will ensure ongoing network partner engagement by working closely with organizations to understand their capacity to take referrals and establish points of contact with key personnel.

Requirements

  • High school diploma or GED.
  • A minimum of 2 years of experience in prevention activities, care navigation, linkage to care, counselling, providing education and/or direct services in a community-based health center or social service environment.
  • Must be willing and able to travel within and throughout the service delivery area.
  • Understanding of and sensitivity to HIV/AIDS, harm reduction, and people who inject drugs population and/or eagerness to learn about these communities.
  • Effective use of motivational interviewing when working with clients.
  • Effective verbal and written communication skills, and a strong comfort level in discussing issues of sexuality, orientation, gender, sex and substance use.
  • Demonstrated cultural competency, including the ability to communicate effectively and work cooperatively with individuals from diverse cultural and ethnic backgrounds.
  • Knowledge of prevention, medical, and social delivery systems.
  • Ability to complete work assignments with minimal supervision, thoroughness, and efficiency.
  • Ability to communicate effectively with community members, organizational leaders, medical providers and others.
  • For positions that require driving a company vehicle, a valid driver’s license, auto insurance and a clean driving record.

Nice To Haves

  • Peer support specialist, a plus.
  • Previous experience working with people who inject drugs preferred.
  • HIV experience in a not-for-profit environment.
  • Experience working in a harm reduction setting.
  • Bilingual.

Responsibilities

  • Work with Prevention staff to identify potential prevention navigation clients.
  • Recruit and enroll clients into the prevention navigation program.
  • Provide support to people who inject drugs who are interested in initiating an individualized plan to access prevention and treatment services.
  • Complete screenings to identify barriers to care and readiness for prevention navigation.
  • Effectively collaborate with Vivent Health Prevention Specialists to provide HIV and hepatitis C testing, counselling and referral.
  • Work with internal and external partners to assist clients in overcoming barriers to services (e.g., safe and stable housing, food services, insurance enrollment, employment).
  • Provide ongoing follow-up to assist clients in achieving their individualized plan.
  • In some cases, in consultation with the supervisor, provide in-home or community-based consultations and assessments.
  • Maintain appropriate client records and timely documentation of services utilizing electronic software.
  • Provide outreach to relevant communities with a focus on people who inject drugs.
  • Establish working relationships with appropriate community resources in the area, including those providing mental health, social services, and drug treatment programs to the target population.
  • Adhere to all agency policies, including but not limited to Confidentiality, Employee Handbook, Health Care Corporate Compliance Plan, Standards of Conduct, training, and other relevant policies.
