About The Position

The Prevention Case Manager/Mentor works directly with at-risk youth within the community to provide guidance, support, and mentorship to help prevent future involvement with the juvenile justice system, substance abuse, and other high-risk behaviors. This role involves assessing the needs of youth, creating individualized plans, and connecting them with resources to promote positive development and life skills. The Prevention Case Manager/Mentor will also engage in outreach efforts and collaborate with schools, families, and community organizations to enhance the overall wellbeing of youth.

Requirements

  • Bachelor’s degree in social work, Psychology, Sociology, Education, or related field (preferred).
  • Previous experience working with at-risk youth or in a case management or mentorship role.
  • Strong understanding of youth development and the challenges faced by at-risk populations.
  • Knowledge of community resources and services available for youth and families.
  • Excellent communication and interpersonal skills.
  • Ability to handle sensitive and confidential information with discretion.
  • Strong problem-solving, conflict resolution, and crisis management skills.
  • Ability to work independently and as part of a team.
  • Compassionate, patient, and empathetic attitude towards youth and their families.
  • Valid driver’s license: must be able to pass an MVR per our insurance.

Responsibilities

  • Mentorship & Support: Provide one-on-one mentorship to at-risk youth, fostering a trusting and supportive relationship to guide personal growth, decision-making, and emotional resilience.
  • Case Management: Conduct comprehensive assessments of youths’ needs, challenges, and goals. Develop individualized prevention plans and support them in achieving those goals, including addressing issues related to education, family dynamics, mental health, and peer relationships.
  • Resource Referral: Identify and connect youth and their families to appropriate community resources such as counseling, substance abuse programs, academic support, job training, housing, and other social services.
  • Community Engagement: Work closely with schools and community organizations.
  • Documentation & Reporting: Maintain detailed and accurate records of youth interactions, progress toward goals, and any interventions provided. Prepare case reports, progress updates, and documentation as required by the organization and funding agencies.
  • Life Skills Development: Facilitate workshops and group activities focused on building life skills, such as conflict resolution, emotional regulation, communication, decision-making, and leadership development.
  • Family Involvement: Engage with the families of at-risk youth to foster supportive relationships and ensure the youth’s needs are being met both within and outside of the family dynamic. Provide family counseling and guidance as appropriate.
