About The Position

The PrEP (Pre-Exposure Prophylaxis) Navigator works closely with the Primary Care Medical Home Coordinator and Medical Providers to ensure the development and expansion and sustainability of the of the PrEP patient navigation and adherence-counseling program at Raphael Health Center. This role supports both clinic-based and community-based initiatives designed to increase access to HIV prevention services. The PrEP Navigator works directly with patients within the health center and throughout the community to encourage access to and coverage for PrEP medications. Responsibilities include conducting community outreach, participating in mobile outreach initiatives, and supporting special projects within the health center as needed to expand awareness and engagement in HIV prevention services. The role also supports HIV/STI testing outreach efforts, care coordination, follow-up services, and collaboration with internal teams and external community partners to improve patient access and retention in care.

Requirements

  • Associates degree preferred or 5 years work experience working in HIV/STI prevent or other community health programs.
  • Certified in HIV/STD testing or willing to become certified within 30 days of hire.
  • Must possess strong communication and written skills, as well as a demonstrated ability to work effectively with diverse staff and populations.
  • Ability to work independently and be self-motivated
  • Ability to work flexible hours.
  • Demonstrated knowledge and competence in the use of software applications: word processing, spreadsheet, presentation, email applications (Microsoft Office preferred).
  • Ability to travel to scheduled events as required, including driving an outreach van.

Nice To Haves

  • Phlebotomy certification preferred.

Responsibilities

  • Performs assessment, plan creation and assisting patients to ensure access to medication and adherence to treatment plan, as well as risk reduction planning
  • Implement an off-site testing program, including movitel outreach efforts and assist with an onsite testing program
  • Develop and implement the Medication Assistance Program for program patients
  • Introduce prevention services ensuring access, follow up, and adherence to PrEP, etc., including supportive services that can enhance harm reduction, behavior change, and maintenance
  • Assist clients as they move through the service delivery system, including follow-up with patient that have fallen out of service
  • Navigate patients through the necessary steps to acquire insurance, as needed, including application assistance
  • Advocate and act as a liaison between the patient and all relevant professionals through the PrEP program that includes prevention, testing, and appropriate treatment that is patient centric
  • Educate clients and others about the HIV disease process and prevention
  • Assist with the elimination of barriers, including but not limited to financial and transportation barriers
  • Collect and report data on program activities
  • Stay informed about current HIV/AIDS trends, prevention efforts and strategies
  • Ability to interact successfully with diverse populations
  • Maintain highest level of client confidentiality; follow HIPAA regulations
  • Attend team meetings and training programs
  • Perform other duties as assigned
