About The Position

Under the supervision of the CRC Director of Ibero-American Action League, the Community Health Worker serves as a key point of contact for individuals seeking assistance from Ibero. This role involves screening, assessing, and providing referrals to Medicaid-eligible clients who may need assistance with housing, food, health care, or other essential human services, while also collecting and maintaining confidential documentation. MUST be Bilingual in Spanish and English, both written and verbal.

Requirements

  • High School Diploma or equivalent required.
  • Associate’s degree in office management, Secretarial or Human Services preferred, or equivalent combination of education, and preferably a minimum of one year of direct experience working with families.
  • Requires basic knowledge of Human Services and community agencies/services available to the public.
  • MUST Bilingual in Spanish and English, both written and verbal.
  • Ability to perform all physical duties required by the position.
  • Proficiency in the use of office equipment including computers, software, copiers.
  • Must possess a valid NYS driver’s license, satisfactory driving record, and transportation.
  • Demonstrate ability to work cooperatively.

Responsibilities

  • Screen and assess clients to determine their needs and eligibility for assistance for housing, food, health care and other essential human services.
  • Complete the intake process using a standard questionnaire form.
  • Collect and maintain accurate client information and documentation.
  • Perform clerical tasks, including data entry, filing, and maintaining organized records.
  • Based on intake forms, determine and provide appropriate community resource referrals as needed for health care, food, clothing, and/or housing.
  • Serve as the initial point of contact by answering all incoming calls promptly and courteously.
  • Greet clients and visitors professionally, ensuring a welcoming environment.
  • Collaborate with other team members to deliver seamless client support and maintain good working relationships with staff from funding sources, related agencies, and other Ibero programs.
  • Provide information regarding program location and agency services to callers and visitors.
  • Verify that the referred individual meets program requirements including having an active Medicaid case.
  • Safeguard confidential information.
  • Maintain sign-in-out logbooks for all visitors.
  • Be available to work outside regularly scheduled working hours when needed.
  • Participate in related agency and community activities.
