About The Position

To implement case management and referral services for inmates pre and post release on a consistent basis in accordance with Inmate Services Division standards, policies, and procedures.

Requirements

  • Bachelor’s Degree from an accredited college in a relevant field or with an Associates License (LSWA) license.
  • Ability to communicate clearly and concisely, in writing and orally
  • Ability to maintain accurate and detailed records
  • Ability to talk to others and to effectively convey information
  • Ability to listen to and understand information and ideas presented through verbal and written communication
  • Beginner skill in the use of personal computers, particularly word processing
  • General knowledge of support agencies such as Alcoholics Anonymous, etc.
  • General knowledge in the effects of drugs and alcohol

Responsibilities

  • To assess basic physical, social, educational, legal and service needs of inmates and coordinate care with other inter-agency and community based providers
  • To effectively communicate and develop positive professional relationships with staff, co-workers, and outside professionals while completing all required documentation
  • To work effectively with community based resources, referral networks, and public/private systems of care
  • To carry a caseload and maintain records on each client on caseload
  • To ensure that case management needs of inmates are met through provision of appropriate referrals for community based services
  • To provide appropriate case management services that match the needs of inmates
  • To develop and facilitate implementation of after-care plan
  • To attend team and case conferences and collaborate with other providers for designated inmates
  • To provide outreach as designated to community-based inmates
  • To foster an atmosphere of cooperation between inmates and staff
  • To observe conduct and behavior of inmates and to report any unusual occurrences to supervisor
  • To report for work on time and appropriately attired
  • To work harmoniously, cooperatively and courteously with others at all times
  • To perform other duties as assigned
  • To maintain regular attendance
