Requirements

  • Bachelor’s Degree in a related field required
  • Two (2) years of experience preferred
  • Excellent writing skills necessary
  • Valid driver’s license and reliable transportation
  • This is a full-time, 40 hour/week position.

Responsibilities

  • Write and implement plans of care to be approved by the consulting agency clinician.
  • Write contact notes, formal monthly reports and discharge summaries.
  • Provide services to family in the home that encompass adjusted parenting education, developmental trauma education, normative adoption development, advocacy, school issues, kinship support, and consultation regarding birth parent contact.
  • Assist the family in finding resources for preventative and supportive services and partner with parents by participating in treatment teams, school teams, coordinated services plan meetings, and other appropriate meetings.
  • Attend PPSP quarterly trainings, a minimum of two additional trainings per year that relate to early trauma or adoption, all VT Consortium trainings and conferences, and all Fostering to Forever classes within their catchment area
  • Visit client homes to meet with caregivers to implement the requirements of the individual’s plan of care and ensure compliance of the case plan.
  • Make diligent efforts to establish and maintain support groups for families formed through adoption and guardianship in assigned catchment area.
  • Testify concerning treatment services in court when subpoenaed.
