Responsibilities

  • Completes residency program requirements and required evaluations as defined by the American Society of Health-System Pharmacists Commission on Credentialing.
  • Documents all residency program activities in electronic residency binder.
  • Achieves program goals as assessed and documented by preceptor evaluations.
  • Completes residency project requirements.
  • Presents project and findings at a regional conference.
  • Consults with patients and healthcare providers to provide information and education regarding drug interactions, side effects, dosage, and proper medication storage to ensure safe use.
  • Contributes to and participates in the professional development of peers, colleagues, and others, including staff education activities, new staff orientation, in-service sessions.
  • Prepares and participates pharmacotherapeutic lectures or operational competency in-service sessions for the health system’s care team.
  • Maintains records, such as pharmacy files, patient profiles, charge system files, inventories, control records, or registries of poisons, narcotics, and controlled drugs; ensures records are maintained accurately in associated databases and in compliance with regulations, policies or procedures.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, appropriateness of drug and dose.
  • Monitors patient microbiology and laboratory indices and makes appropriate dosing changes when necessary.
  • Monitors drug therapy to ensure compliance with CMS, Quality of Care core measures, and any other applicable regulatory, professional and organizational guidelines.
  • Participates in scheduled in-services, educational presentations, computer based training, and staff meetings to ensure ongoing understanding of and adherence to professional standards, regulatory requirements and organizational policies and practices.
  • Provides clinical pharmacy services, including but not limited to, pharmacokinetic consults, renal dosing, parenteral nutrition management, anticoagulation management, neonatal/pediatric/geriatric dosing management, route optimization, antimicrobial stewardship, and reviewing medications at transitions of care.
  • Reviews prescriptions and medication orders, checks for appropriateness and provides information needed to properly and safely administer the medication.
  • Recognizes orders that are inappropriate and takes responsibility to assure that the order is modified promptly and correctly. Documents interventions.
  • Supports cost-effective formulary management initiatives.
  • Participates in formulary protocol and policy development and implementation.
  • Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
