Partner with Community Health Network and Deaconess Health System – IHCI The Innovative Healthcare Collaborative of Indiana (IHCI) is a joint venture between Community Health Network and Deaconess Health System. Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system. Make a Difference As a Post-Acute Care Coordinator LPN on the IHCI Team, you will provide collaborative, planning, referral and monitoring of treatment and support services to adults with severe mental illness, persons with chronic addictions, or children with severe emotional disturbances and their families. The Post-Acute Care Coordinator LPN will coordinate collaborative patient care, case management and/or coordination experience in a setting that includes a collaborative, iterative process of assessing, diagnosing, planning to achieve mutually agreed upon patient goals, implementing interventions, and evaluating progress toward goals. The Post-Acute Care Coordinator LPN works directly with patients and providers in a variety of settings with both health systems and independent providers that are in our network to identify and engage high risk patients. Monitors home equipment and reviews data daily. Escalates concerns from patients / equipment monitored to appropriate physician. May work closely with specific patient populations (e.g., end-stage renal disease patients, behavioral health patients) in a variety of settings and with specialty providers in both health systems and independent clinics that are in our network. Investigating psychological and social determinant patient barriers and designing appropriate referral channels to assist in closing needs gaps.

Requirements

  • Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience required.
  • Licensed as a Practical Nurse by the Indiana Professional Licensing Agency required.
  • Current Indiana Driver's License with valid vehicle insurance required.

Nice To Haves

  • Two years of nursing experience preferred but not required.
