About The Position

As a Care Manager RN for Post-Acute Care IHCI, you will have the opportunity to make a profound impact on the lives of people living with complex and/or chronic conditions. This job is responsible for supporting the healthcare team towards a smooth transition from one level of care to another in support of the patient/family. To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration and functions of case management, utilization review and management and discharge planning. The role is responsible for managing post-acute services for patients across skilled nursing facilities (SNFs), including communicating and monitoring the expected length of stay for specific patients. Working autonomously, the position manages patients within the Accountable Care Organization (ACO) across multiple post-acute care facilities and collaborates closely with facility staff to coordinate care plans and address patient needs.

Requirements

  • 2 year / associate degree in nursing (required)
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) (required)
  • 3+ years: nursing experience, preferably in Population Health management, Case Management, or Post-Acute Care. (required)

Nice To Haves

  • Certified Case Manager (CCM), Accredited Case Manager (ACM), or equivalent certification.
