About The Position

The Population Health Program Manager is responsible for the development and implementation of a Population Health Program within NACC. Program implementation will be based on the development of a Population Health Plan based on population outcomes in clinical care, social risk factors, and patient experience. This role also manages clinical programs such as cancer screening, medical nutrition therapy, chronic disease grant management, and chronic disease quality initiative support.

Requirements

  • Bachelors Degree in clinical or public health field
  • 5 to 10 years of related experience and/or training, or equivalent combination of education and experience

Responsibilities

  • In coordination with Medical Team, develops Population Health Workplan that guides program development, implementation, and evaluation.
  • Leads the implementation of the Population Health Workplan.
  • Develops strategies to evaluate social risk factors of NACC's patient population and surrounding communities and implements tactics that target barrier reduction.
  • Manages staff that provide population health, chronic disease and nutrition management care, and education.
  • Supervises and leads the population health, cancer screening, and chronic disease management program.
  • Provides guidance and coordinates chronic disease trainings for medical staff.
  • Assists with grant searches, proposal writing and submission for population health and chronic disease management/prevention projects and manages deliverables (invoices, program implementation/maintenance) associated with non-research-based grants.
  • Collaborates with community partners to develop culturally relevant chronic disease management programs to meet the needs of the clinic patient population.
  • Assists with population health quality improvement projects for chronic disease and nutrition such as diabetes, cardiovascular disease, hypertension, and asthma.
  • In coordination with Medical Leadership, develops policies, protocols, best practice documentation, and care team training within population health program.
  • Manages ADA Recognized DSME Program and compliance and AHA Target Blood Pressure program.
  • Compilation of monthly chronic disease program reports
  • Assists with other duties as assigned.
