Boasting 24 state-of-the-art exam rooms and an on-site phlebotomy lab, the clinic is designed for convenience and efficiency. With a dedicated team of 14 providers, 2 licensed social workers, 2 registered dietitians, and exceptional clinical staff, the clinic offers a full spectrum of services, including preventive care, chronic disease management, women’s health, pediatrics, immunizations, and diagnostics. Duke Primary Care Croasdaile is committed to delivering comprehensive, compassionate care to meet the diverse needs of families in the community. Location: 1821 Hillandale Road Suite 24B, Durham, NC 27705 Schedule: Monday - Friday, 8am - 5pm The Population Health Nurse partners with pediatric patients with complex health needs, delivering care and education through a dynamic blend of educational telehealth visits and care coordination. This role guides children and their families in managing chronic illnesses, develops and coordinates individualized care plans, and ensures connection to the right resources and support. As an integral member of an interdisciplinary team, the nurse helps families navigate care transitions, empowers them with knowledge and tools, and drives improved, patient-centered health outcomes.
