Summary The Population Health Coordinator will manage multiple, complex projects related to quality, practice transformation, and value-based reimbursement. This position will closely work with interdisciplinary teams involved in all stages of project development. The Population Health Coordinator will be accountable for the detailed planning of tasks and resources needed to meet project objectives. The Population Health Coordinator also monitors and prepares reports to ensure work is progressing to meet the deliverables on time and budget.

Requirements

  • High school diploma or GED equivalent
  • One (1) year of experience in healthcare

Nice To Haves

  • Five years' experience in a healthcare organization
  • Experience with Epic application and health record
  • Experience and understanding of Microsoft Office
  • Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
  • Bilingual English / Spanish - complete comprehension, literacy, and fluency in both languages

Responsibilities

  • Communicates in a professional, clear, effective, and compassionate way with patients, visitors, and coworkers, on the telephone, and with written communication.
  • Maintains a 95% or greater empanelment rate, this is accomplished through daily, weekly, and monthly tasks.
  • Coordinates and completes the Annual Reassignment Project to reallocate graduating class patient panels to new and current providers.
  • Utilizes various data sources in the EHR including Electronic Clinical Quality Measures (ECQMs) and UDS measures to plan, complete and monitor the majority of the intervention and outreach projects.
  • Transitions raw data from a variety of reports into meaningful data that providers and associates can understand and use to effect change.
  • Collaborates with leadership to maintain the effectiveness of incentives and projects tied to EQCMs or Quality Improvement.
  • Available to as required and as scheduled and report to work on time regularly and reliably.
  • Documents properly and accurately within the electronic medical record: recording patient examination, treatment, and test results.
  • Maintains patient confidentiality and protects medical office operations by keeping information confidential; cautioning others regarding potential breaches.
  • Participates in huddles daily, per huddle procedure.
  • Attends required staff meetings, standard work meetings, and associate training events
  • Other duties as assigned.
  • Available to work in-person for clinic coverage if needed
  • Complies with all policies, procedures, and standard work.
  • Complies with the environment of care, patient safety and infection control procedures.
  • Maintains patient confidentiality, in accordance with HIPAA guidelines.
  • Pass all competencies initially and annually on time.
