About The Position

In collaboration with the Director, this individual will be responsible for conducting data analysis, creating reports on program outcomes, developing and maintaining program performance metrics, identifying areas for improvement in existing programs, making recommendations for change, and working with clinical teams to develop new programs and initiatives to achieve organization goals. The Population Health Analyst will also coordinate with other departments and stakeholders to ensure program success.Job Description:Essential Duties & Responsibilities including but not limited to:1. Gathering, cleaning and analyzing population health data from various sources, including electronic health records (i.e. Epic, Athena, eCW), claims data, surveys, and other relevant sources (i.e. Arcadia).2. Identify patterns, trends, and correlations in data to assess the health needs and risks of different populations.3. Support the development of population health strategies and interventions based on data analysis andevidence-based practices.4. Collaborate with internal and external stakeholders to collect, validate, and analyze data to assess theeffectiveness of population health programs.5. Prepare and present reports, dashboards, and visualizations that communicate key findings andrecommendations to stakeholders.6. Monitor and evaluate the performance and impact of population health initiatives/programs, includingtracking key performance indicators and outcomes.7. Effectively communicating findings and recommendations to diverse audiences, including clinical staff,community partners, and policymakers.8. Leading and managing analytic projects, including vendor partnerships and the development of analytictools.9. Support, train, and guide cross-functional team members on accurate data extraction and analysis, ensuringalignment with performance goals and programmatic strategies to drive informed decision-making andcontinuous improvement.10. Lead meetings focused on performance improvement and program development, providing strategicinsights, facilitating stakeholder engagement, and driving alignment on goals, initiatives, and next steps.11. Stay current on population health trends, best practices, and emerging technologies to support continuousimprovement and innovation.1. Requirements

  • Bachelor's degree in healthcare management, public health, health informatics, epidemiology, statistics, or a related field.
  • Proven experience in population health analysis and program evaluation, including population management programs.
  • Strong analytical skills and proficiency in data analysis tools and software.
  • Knowledge of statistical methods, epidemiology, public health principles, and health disparities.
  • Strong leadership and management skills.
  • Collaborative and team player.
  • Knowledge and understanding of value based care, risk contracts.
  • Understanding of process improvement tools and techniques.
  • Facility with Microsoft Word, Excel, Power Point.

Nice To Haves

  • Master's degree preferred.
  • Experience with Epic, Epic Healthy Planet and Arcadia preferred.
