KFI has an exciting opportunity for a full time Planning Coordinator. This key role is responsible for ensuring that the needs and dreams of people supported are accurately reflected in KFI generated documentation, including but not limited to, Service Implementation Plans and daily documentation. The Planning Coordinator also promotes the principles of SRV (Social Role Valorization), and acts as the primary liaison with DHHS and Community Case Managers. This is accomplished through close collaboration with the person, their KFI Support Coordinator, family members, Community Case Managers, KFI staff, members of the community, and state agency personnel. The Planning Coordinator will be responsible for arranging and facilitating pre-planning and discovery meetings, writing Service Implementation Plans and entering information into state systems. To achieve this, the Planning Coordinator will maintain a schedule of regular in-person visits with all people on their caseload to develop and maintain an understanding of each person’s support needs. Our Planning Coordinators collaborate with the team’s Support Coordinator to ensure that each person's needs, goals and desires are accurately reflected in all documentation. They also assist with monitoring documentation and progress towards identified goals. Planning Coordinators must maintain working knowledge of State and Federal regulations pertaining to support provided and ensure compliance with Home and Community Based Supports (HCBS) regulations by completing forms with each person on caseload and updating annually or as required. Planning Coordinators also engage in management meetings, discussions and activities relating to the development, improvement, and expansion of KFI services. The position may be based in either the Lincoln or Millinocket office.
