About The Position

Responsible for the planning, documentation, distribution and implementation of Programming/Services for those attending the Prevocational program, and ensuring all regulations are followed. Schedules meetings for and ensures completion of Life Plans. Ensures completion, routing and implementation of Staff Action Plans, LCEDs, DDP2’s and all protocols. Assumes a leadership role in the development, documentation and implementation of vocational plans for individuals on caseload. Assists with review of daily tracking, monthly summaries, completion and review of Annual Individual Eligibility Evaluation’s (IEE’s), Annual Prevocational Community assessments, physical plant needs, vehicle needs, emergency evacuation drills, and water temperature checks as needed. Empowers the people we serve to gain independence by promoting opportunities, services and attitudes that develop, enhance and encourage self-sufficiency and employability.

Requirements

  • Associates’ Degree in human services, education, psychology or related
  • Valid NYS license and meet the Agency driving policy requirements
  • Complete and maintain SCIP R Core certification
  • Effective communication with people we serve, their supports and staff
  • Ability to assist the people we support to pursue and obtain their personal goals
  • Ability to work effectively without direct supervision to ensure completion of mandatory daily and monthly documentation
  • Ability to effectively balance multiple tasks and ever-changing priorities

Nice To Haves

  • Bachelor’s Degree in human services, education, psychology or related
  • Experience working with adults with developmental and/or cognitive disabilities
