About The Position

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reentry and recovery housing throughout the program area to assist participants with reentry and recovery resources. The Placement Coordinator manages the referral, assessment, and placement of justice-involved individuals into appropriate community-based programs and services. This in-person role coordinates with CDCR, Parole Agents, and subcontracted providers, ensures accurate recordkeeping and data entry, monitors program compliance, and facilitates communication between stakeholders. The role alsosupports staff and new hires through training and technical assistance, conducts presentations to community agencies, and travels to program sites to oversee placements, maintain relationships, and conduct presentations, ensuring participants successfully access the services they need.

Requirements

  • High school diploma or equivalent. Two years of college education in the field of Criminal Justice, Psychology, Sociology, or related field preferred.
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver’s License and access to registered and insured transportation.
  • Minimum of two (2) years’ experience working with criminal justice and/or substance abuse treatment programs.
  • Experience working in a customer service driven environment.

Nice To Haves

  • Experience using an electronic health record system preferred.
  • Experience with governmental behavioral health contractual and/or sub-contractual standardspreferred.

Responsibilities

  • Receive referrals for parolees from in-prison programs and DAPO Parole Units.
  • Proactively link clients to Community-Based Providers (CBPs) based on treatment needs/level of care.
  • Submit service authorizations to CBPs.
  • Assess, schedule, and coordinate placements of project referrals into appropriate community placements.
  • Coordinate transportation with Transportation Coordinator.
  • Match inmates/parolees to the appropriate level of care within a network of subcontracted community-based providers.
  • Screen participant profiles for barriers to treatment delivery.
  • Receive transitional plans from CDCR and county institutions.
  • Conduct daily communication with Parole Agents.
  • Conduct presentations to community agencies to increase referral pools.
  • Identify community capacity needs.
  • Revise and submit placement requests.
  • Notify AORs of participant placements.
  • Enter placement notes into data and/or ARMS Data system.
  • Coordinate with internal and external resources (SAP staff, Parole, CTP, community providers, County Jails, STOP staff) to produce placement notes.
  • Serve as an Incident Reporting designee as needed.
  • May coordinate client release dates with transportation services.
  • Create, open, update, close, and archive client files according to department procedures.
  • Assist in establishing and maintaining an effective and efficient records management system.
  • Ensure participant files meet contractual compliance.
  • Maintain and comply with confidentiality requirements.
  • Conduct file compliance and reconciliation.
  • Maintain accurate records by entering documentation into electronic systems according to HealthRIGHT 360 guidelines.
  • Keep track of received data and source documents.
  • Prepare, sort, and identify data to be entered.
  • Contact preparers of source documents to resolve questions, inconsistencies, or missing data.
  • Compile, sort, and verify accuracy of data to be entered.
  • Review and correct information entered.
  • Generate reports and respond to inquiries regarding data.
  • Maintain and update bed counts and reporting spreadsheets.
  • Submit reports to QA/Fidelity Supervisor, Case Manager Supervisor, Program Director, or CDCR as needed.
  • Perform general clerical duties such as typing, answering phones, filing, and processing forms.
  • Conduct presentations to community agencies to technically assist CBP staff.
  • Attend required trainings and meetings.
  • Assist in training new hires.
  • Complete assigned training in a timely manner.
  • And perform other duties as assigned.
