About The Position

The Placement Case Manager is responsible for ensuring clinical oversight for coordinating and scheduling respite placements for children ages 3 – 18 with our certified resource/respite homes providing respite care. The role participates as part of the team supporting children in care and their biological/foster placements. The position is responsible for assisting with respite parent supervision, respite home safety checks, and facilitate/attend program support and attends training meetings along with respite providers. Responsible for collection, clinical review, and upload of respite reports to client files. Engaged with potential, new, and existing resource parents. Partner with client caregivers and clinical team coordinating respite intake process, ongoing eligibility, and respite matching. Participation required of resource home recruitment and after hours on-call rotation.

Requirements

  • Bachelor’s Degree in Social Work or allied field from an accredited college or university and two years in the care and treatment of children and young adults
  • Attained age 21 years or older before first day of employment in position (ORR)
  • Basic computer skills in word processing, databases, spreadsheets, and email.
  • Able to pass a comprehensive criminal history and child abuse background investigation
  • For positions that require driving:
  • Hold a valid drivers’ license. Must be licensed in Oregon or Washington within 30 days of hire
  • If driving personal vehicle, must maintain minimum auto insurance policy
  • Able to work shift hours and days, as assigned. Position requires occasional support to on-call after hours support line, recruitment events participation weekend and evening work.

Nice To Haves

  • Master’s in Social Work or related field
  • Supervision of staff or lead experience
  • Experience in residential treatment setting
  • Bilingual in Spanish and English, written, read and verbal. Must clear a language proficiency test for Spanish.

Responsibilities

  • Schedule respite placements of client with appropriate and available respite homes
  • Coordinate pre-respite meetings and post-placement debriefings, as needed
  • Timely and effective communication to client’s caregivers and mental health therapists regarding access to respite services, and inquiries of program eligibility
  • Understand client intake/referral paperwork for eligibility for respite services for those authorized for services
  • Provide training for respite parents and support facilitation of PCRC monthly support nights
  • Ensure respite placements and requests are entered in client electronic record
  • Responsible for collection, clinical review, uploading of respite reports, and incident reports to client files
  • Ensure accuracy of monthly billing reports for respite provider monthly invoices
  • Assess respite provider’s capacity to be trauma-informed in working with clients with severe mental health
  • Conduct respite home visits to verify safety standards are maintained and submit written documentation of the visit.
  • Participate in resource home recruitment events and program outreach for Morrison Respite programs
  • Model safe language and demonstrate appropriate boundaries with peers, resource parents and clients
  • Intervene and provide support when staff, families, or clients demonstrate unsafe language or behaviors
  • Maintains confidentiality and appropriate professional boundaries
  • Demonstrate a commitment to developing a thorough knowledge and application of the Sanctuary model and other organizational policies & practices.
  • Participate in staff development, in-services, and training related to workplace effectiveness; model professional behaviors and implement improvements to business practices with awareness of their impact in a trauma-informed behavioral healthcare organization.
  • Other duties as assigned to ensure successful operation of program/department
