About The Position

Chaparral Medical Group is seeking passionate and team-oriented Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our fast-growing, mission-driven care team. Based in our family of primary care clinics in the Pomona Valley region, you’ll play a key role in delivering high-quality, compassionate care to adult patients across a broad range of needs. You’ll work alongside a collaborative team of clinicians, benefit from integrated virtual support, and be empowered to make a real impact on the health of our communities.

Requirements

• Completed an accredited NP or PA program with current national certification (required)
• Licensed to practice in the state of California (required)
• In the past 5 years, you have:
• 2+ years of experience as an Advanced Practitioner in an outpatient primary care setting, OR
• 1+ year in outpatient primary care and either 1 year in a primary care fellowship or 1+ year in urgent care (required)
• Strong diagnostic and clinical decision-making skills
• Excellent communication, patient education, and interpersonal abilities
• Comfortable working in a team-based environment and adapting to a fast-paced clinic setting

Responsibilities

• Manage a patient panel, delivering care across acute, chronic, and preventive visits
• Provide in-person care at a clinic site with occasional telehealth appointments as needed
• Collaborate with in-office teammates during daily huddles and virtual clinical team check-ins
• Perform in-office procedures aligned with your clinical training and credentials
• Support patients in their care journey by coordinating referrals and follow-ups with specialists and facilities
• Maintain accurate and timely patient documentation within our electronic medical records system
• Contribute to a team culture of quality, collaboration, and continuous improvement
