We are seeking Licensed Nurse to join our team in our Physician Support Nurse role servicing our patients throughout the Augusta area. Physician Support Nurse role will provide direct clinical and care coordination support to physicians within the care group. The PSN ensures that physicians are equipped with accurate patient information, coordinated follow-up plans, and streamlined communication with the interdisciplinary care team. This role combines clinical expertise, patient engagement, and strong organizational skills to improve care quality, support population health goals, and enhance physician effectiveness in delivering value-based care. This position will be working in the Augusta office daily. This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).   • Serve as the dedicated nurse supporting the physician on the care group.
  • Prepare and reconcile patient charts, ensuring all test results, care gaps, consents, and documentation are accurate and available prior to visits.
  • Anticipate physician needs during patient care and provide clinical assistance as appropriate.
  • Support diagnosis accuracy, documentation standards, and quality measure completion.
  • Facilitate effective communication across the care group to align on patient care plans.
  • Perform intake, vital signs, and medication reconciliation; document patient histories in the EMR.
  • Provide patient education on care plans, conditions, medications, and self-management strategies.
  • Monitor ADT alerts (admissions, discharges, transfers) for assigned patients; ensure timely physician follow-up.
  • Coordinate with specialists, hospitals, and post-acute facilities to support smooth transitions of care.
  • Schedule follow-up visits and preventive services to close care gaps and maintain care continuity.
  • Support quality performance by tracking and closing care gaps, improving documentation, and promoting preventive services
  • Manage referrals, labs, imaging follow-up, prior authorizations, and DME orders
  • Licensed nurse (LPN or RN) license required. License must be in good standing.
  • Minimum 2 years of clinical experience in ambulatory/primary care, community health, or care coordination.
  • Current CPR/BLS certification.
  • Proficiency with EMR/EHR systems and care-coordination workflows; able to work buckets, track ADT alerts, and document timely and accurately.
  • Demonstrated skills in chart preparation, medication reconciliation, vital signs, patient education, and documentation to support provider efficiency and quality.
  • Strong interpersonal, written, and verbal communication skills; ability to facilitate multidisciplinary follow-through (IDG action items, referrals, labs, imaging, DME, prior auths).
  • Must be proficient in utilizing technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Ability to read and communicate effectively.
  • Strong written and verbal skills.
  • Basic computer knowledge.
  • Ability to manage and demonstrate effective leadership skills.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Ability to foster a cooperative work environment.
  • Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
