Requirements

Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited medical school
Completion of accredited Residency Program
Board-certified or board-eligible in pediatrics
License or eligible for licensure in Indiana
Active and unrestricted DEA license or ability to obtain prior to start
