About The Position

Ascension Illinois is looking for a Board Certified/Board Eligible Palliative Care Hospice Physican for our Palliative Care and Hospice Department. This is a great opportunity for the right candidate to partner with Ascension Illinois to provide medical services to patients. Position Highlights: Physician will provide consultation services, Monday-Friday Busy and well-respected Palliative Care Service Looking for physicians that thrive in a multidisciplinary teams and are comfortabe working alongside Nurses, Nurse Practitioners, Social Workers and Chaplains Opportunities to participate in quality committees and teach medical trainees Primary location is Elk Grove Village, IL

Requirements

  • Physician MD/DO credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Credentials are verified by local staff credentialing office.
  • Advanced Life Support preferred. American Heart Association or American Red Cross accepted. Verified by local credentialing office.
  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.

Responsibilities

  • Provide medical services to patients in an assigned area.
  • Evaluate and treat patients with appropriate medical diagnostic and treatment skills. Coordinate discharge planning with case management.
  • Recommend, participate in, and work to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
  • Accept patient referrals from other physicians. Act as admitting, attending and/or consulting physician for patients who require hospitalization for primary medical diagnoses, depending on needs of patients, medical staff and hospital.
  • Participate in academic programs (eg medical education and research) as requested.
  • Provide appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.
  • Participate in inpatient and outpatient consultation for specialty, which may include on call schedules.
