Hospice & Palliative Care Physician | Full-Time Location: Shelton, Connecticut A well-established, community-focused healthcare organization in the Shelton, Connecticut area is actively seeking a full-time Hospice & Palliative Care Physician who can also provide Hospitalist coverage. The organization maintains an academic affiliation with the Yale School of Medicine, offering opportunities for teaching, mentorship, and professional growth. Position Overview Provide comprehensive hospice and palliative care services in inpatient, outpatient, and home-based settings Conduct goals-of-care discussions, advance care planning, and symptom management Manage pain and complex symptom control for patients with serious or life-limiting illnesses Provide hospitalist coverage, including inpatient admissions, daily rounding, and coordination of care Collaborate closely with interdisciplinary teams including nursing, social work, chaplaincy, and case management Support transitions of care between hospital, hospice, and community settings Participate in shared call coverage Maintain timely and accurate documentation in the EMR Opportunities to engage in quality improvement and academic initiatives

Requirements

  • Board Certified or Board Eligible in Hospice & Palliative Medicine, Internal Medicine, or Family Medicine
  • Eligible for Connecticut State medical licensure
  • Active or eligible DEA registration
  • Strong clinical, communication, and interdisciplinary team collaboration skills
  • Commitment to compassionate, patient-centered serious illness care
