The physician will serve as the primary care provider for a dedicated panel of geriatric patients. Patients are located in long-term care facilities (ALFs, ILFs, SNFs) and private homes. Dedicated schedulers put together daily schedules / routes for the providers to ensure efficiency and appropriate time for every visit. Specific duties of the role include, but are not limited to: - conduct health assessments to identify patient needs - develop personalized care plans - regularly evaluate patient conditions and responses to treatment - adjust treatment plans as necessary - perform minor procedures as required and as settings permit - educate patients and families on health management, disease prevention, etc. - provide guidance on lifestyle modifications and self-care practices - work with other providers, nurses, and healthcare professionals to coordinate care The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
