Senior Doc, part of Brigade Health, is seeking a Primary Care Physician (MD / DO) to deliver mobile primary care to older adults in home and senior living settings. Requirements

  • MD or DO degree from accredited medical school
  • Board Certified or Board Eligible in: Internal Medicine Family Medicine Geriatrics
  • Active State Medical License or ability to obtain
  • DEA License

  • Experience in: Home-Based Primary Care Skilled Nursing Facilities (SNF) Assisted Living Geriatrics Post-Acute Care

Responsibilities

  • Provide mobile primary care services to geriatric patients in homes, assisted living, and SNF settings
  • Manage and grow a patient panel
  • Deliver care for chronic conditions, dementia, and complex geriatric patients
  • Collaborate with interdisciplinary teams to coordinate care
  • Develop relationships with facility partners and caregivers
  • Maintain timely EMR documentation
  • Participate in quality improvement and clinical leadership initiatives
