Requirements

Be a Primary-Care Physician, Family Practice Physician, or Internal Medicine Physician
Ability to practice medicine to the full scope of your ability
Commitment to value-based care and patient-focused outcomes

Responsibilities

Practice preventive and coordinated primary care
Focus on patient outcomes, quality measures, coordination of care, avoidance of unnecessary testing, ER visits leading to hospitalizations, and patient experience
Deliver exceptional care that is patient-focused, value-based, and outcome-driven
Utilize clinical, business, and technology resources to focus on patient care
Participate in new quality programs, including Virtual Care Clinic and Telehealth services, to improve patients' overall health care
