Requirements

  • Unrestricted Arizona Physician Assistant OR Nurse Practitioner license or ability to obtain prior to start date
  • Physician Assistant national certification through NCCPA; OR National Nurse Practitioner certification through AANC or AANP
  • Current DEA registration or ability to obtain prior to start date
  • Current BLS certification or ability to obtain prior to start date

Nice To Haves

  • Experience in providing patient care under a Value Based Care Model
  • Experience in outpatient Primary Care
  • Knowledge of HEDIS measures, CAHPS, HOS, HCC Coding documentation
  • Proficient in Medicare Annual Wellness visits
  • Proficient in EMR systems & technology (EPIC Preferred)
