About The Position

The Osborn is a premier senior living community in Rye, NY, dedicated to providing exceptional rehabilitation and wellness services for older adults. Our team delivers personalized, compassionate care that helps residents regain strength, mobility, and independence. We are seeking a New York State–licensed Physical Therapy Assistant (PTA) to join our Senior Living Rehab team.

Requirements

Active New York State Physical Therapy Assistant (PTA) License issued by the NYS Education Department.
Completion of an accredited PTA program recognized by NYSED.
Current NYS registration in good standing.
CPR/BLS certification.
Strong interpersonal communication and clinical documentation skills.

Nice To Haves

Experience in senior care, long‑term care, or post‑acute rehab preferred.

Responsibilities

Deliver therapy treatments under the direction of a licensed Physical Therapist in accordance with NYS practice guidelines.
Assist residents with therapeutic exercises, gait training, balance improvement, transfers, and functional mobility tasks.
Accurately document treatments and progress per NYS and facility standards.
Observe and communicate changes in resident condition to the PT and interdisciplinary team.
Educate residents and families on exercises, mobility techniques, and safety practices.
Maintain a safe environment in the therapy gym and resident care areas.
Collaborate with nursing, OT, speech therapy, and other team members to support resident goals.
