About The Position

Each patient that is referred to the Physical Therapy Program by a CTC physician will receive a complete evaluation by a licensed physical therapist. Each patient will then receive treatment from a licensed physical therapist and/or physical therapist assistant according to an individualized treatment plan. All services are to be recorded in the Electronic Health Record (EHR).

Requirements

• Understanding of substance and mental health issues required.
• Interpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members required.
• Computer proficiency inclusive of ability to access, enter, and interpret computerized data/information required.
• Basic Life Support certification required (BLS).
• Associate degree from accredited program required
• Arizona Physical Therapist Assistant license with ability to work under general supervision required
• Must be at least 21 years of age.
• Demonstrated interest in working with an underserved population.

Responsibilities

• Treat assigned patients according to plan established by supervising physical therapist
• Document each visit in the Electronic Health Record (EHR) within 24 hours of treatment
• Document and discuss progress toward established goals with supervising physical therapist
• Participate in team discussion surrounding patients’ plan of care, goals and continued therapy
• Communicate with supervising physical therapist in person or by phone
