PT - Physical Therapist (Per Visit)
About The Position
Abcor Home Health is a family-owned agency that cares for both our patients and our team. We’re looking for a dedicated and visionary Physical Therapist to lead rehabilitative care and help patients achieve their highest level of function at home. If you believe in holistic recovery, value clinical autonomy, and want to shape care plans in a supportive environment, this role is for you. 📍 Position Details Location: Rockford and nearby areas (within 30 minutes preferred) Training: Leadership and administrative onboarding in Arlington Heights. Visits: In-home evaluations and treatments. Develop plans that make a real-world difference. Experience: Home Health experience, including evaluation and oversight, is required.
Requirements
- A Doctorate or Master’s degree in Physical Therapy from an accredited program.
- A valid Illinois PT license in good standing.
- Significant experience in Home Health, including OASIS knowledge and case management.
- Strong leadership, clinical decision-making, and critical thinking skills.
- The ability to work autonomously while guiding a team.
- Exceptional communication skills and a commitment to patient-centered, outcomes-driven care.
- Valid Illinois PT License (required)
- Valid Driver’s License and reliable, insured vehicle (required)
- CPR Certification (required)
Responsibilities
- Conduct comprehensive initial and ongoing assessments of patients’ functional abilities and limitations.
- Develop, implement, and oversee individualized plans of care for a diverse patient population.
- Provide skilled treatment including therapeutic exercise, manual therapy, balance training, and patient education.
- Supervise and collaborate with Physical Therapist Assistants (PTAs).
- Educate patients, families, and caregivers on disease management, fall prevention, and adaptive equipment.
- Collaborate closely with physicians, nurses, and other members of the interdisciplinary team.
- Document according to Medicare/insurance guidelines and oversee the therapy plan’s progression.
