About The Position

Abcor Home Health is looking for a compassionate and skilled Physical Therapist Assistant to join us in delivering outstanding rehabilitative care at home. If you believe in restoring mobility, value one-on-one patient interaction, and want to grow professionally in a supportive environment, this role is for you. 📍 Position Details Location: Rockford and nearby areas (within 30 minutes preferred) Training: Comprehensive onboarding in Arlington Heights (length tailored to your experience) Visits: In-home patient visits. Focused, effective sessions tailored to each care plan. Experience: Home Health experience and current clinical skills required.

Requirements

An Associate’s degree from an accredited PTA program.
A valid Illinois PTA license in good standing.
Ability to work independently with strong time management skills.
Compassion, patience, and the adaptability to work in diverse home settings.
Excellent interpersonal and communication skills.
Valid Illinois PTA License (required)
Valid Driver’s License and reliable, insured vehicle (required)
CPR Certification (required)

Nice To Haves

Experience in Home Health is a strong plus.

Responsibilities

Implement prescribed physical therapy programs under the direction of a licensed Physical Therapist.
Provide hands-on therapeutic exercises, gait training, and mobility activities.
Utilize modalities as directed (e.g., heat, cold, ultrasound).
Educate patients and caregivers on home exercise programs and safety techniques.
Document patient progress and communicate effectively with the supervising PT and care team.
Motivate patients and help them achieve functional goals in their home environment.
