Physical Therapist Assistant (PTA) – Home Health – Rockford & Surrounding Areas

Details
Territory: Rockford & nearby areas (within ~30 minutes preferred)
In-home visits
Training in Arlington Heights (based on experience)
Responsibilities
Deliver PT programs under supervising Physical Therapist
Assist with therapeutic exercises, gait training, and mobility
Use modalities as directed
Educate patients and caregivers on home exercises and safety
Document progress and communicate with care team
Requirements
Associate’s degree from accredited PTA program
Active Illinois PTA license
Home Health experience preferred
Valid Driver’s License and reliable vehicle
CPR certification
Strong communication and time management skills

Requirements
