About The Position

Provides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age-appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice.

Requirements

• Degree from an American Physical Therapy Association ( APTA ) accredited program required.
• Minimum one (1) year of experience in health care setting performing similar work activities preferred.
• Licensed as a Physical Therapist by the state of New Hampshire Office of Licenses Allied Health Professionals required.
• BLS, CPR required.
• Documentation on laptop computer, basic typing skills.
• Participate in rotating weekend commitment.
• Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities required.
• Proof of auto mobile insurance required, and minimum coverage met.
• Motor vehicle background report will be conducted.

Responsibilities

• Plans, develops, and implements physical therapy care plan appropriately.
• Regularly re-evaluates and updates appropriateness of care plan and physicians’ orders.
• Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved.
• Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes.
• Orients and supervises PTA’s according to State of NH guidelines and VNA policy.
