For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned. This position also assists in the supervision of assistants, certified home health aide, students, interns as assigned. Additional duties include scheduling patients and staff, participation in discipline specific projects i. e. chart audits, QI data collection, peer review program, student program and CHHA training. Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care. Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials, and establishing treatment priorities. Provides an organized and efficient patient treatment, which includes advising, educating and counseling the patient. Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan. Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer. Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients Meets agency productivity standards and uses work hours efficiently. Seeks and accepts assistance from team members in treatment planning and intervention. Completes assessments, and submits daily progress notes, discharge summaries and other required forms in a timely manner to prevent interruptions in patient care. Maintains patient documents that allow other therapists or assistants to effectively treat the patient. Attends and participates in agency meetings and committees as assigned. Communicates the benefits of therapy effectively to patient and family. Appropriately negotiates the patient participation in treatment, treatment times and treatment plan. Monitors patient response to therapy. Communicates concerns promptly to physician and/or manager. Documents clearly patients/family ability to perform home exercise program. Utilizes and directs support staff (assistants and CHHAs, CNAs) appropriately and in accordance with agency and regulatory guidelines. Schedules/follows the schedule for patient according to proper procedure and informs appropriate persons of scheduling changes. Uses equipment and supplies in a proper and safe manner and notifies supervisor of equipment maintenance issues and/or need for supplies. Returns equipment and supplies following treatment. Promotes professional growth by attending, presenting formal and informal educational experiences, in-services, conferences and seminars and/or reviewing professional literature. Monitors and completes required written co-signatures for assistants and support staff on an ongoing basis. Communicates patient treatment plans, changes in physician' s orders and advancement of patient treatment techniques effectively to assistants and support staff. Follows-up and responds to memos, requests and voice mail in a timely manner. Participates in the student clinical education program and functions as clinical resource for students. Promotes agency programs and activities within and outside of the agency. Adapts to environmental changes and is able to apply change to a growth, learning and development opportunity. Demonstrates a willingness to take on projects and other assignments. Maintains and meets agency and professional standards and practices according to the requirements of the accrediting and licensing agents. Performs other duties assigned. Maintains compliance with Medicare regulations and completes functional re-assessments at the appropriate time points
