We are growing and seeking an experienced, compassionate Physical Therapist (PT) to join our team on a per diem basis in the Inland Empire area! Why Join Bella Terra? Requirements
- Active California Physical Therapy License
- Minimum 2 years of physical therapy experience (home health experience preferred)
- Current CPR certification
- Reliable transportation and valid driver’s license with insurance
- Strong verbal, written communication, and organizational skills
Responsibilities
- Provide physical therapy services according to a physician’s plan of care.
- Assess and evaluate patient’s therapeutic/rehabilitative/functional status and participate in developing the total plan of care.
- Evaluate home environment for safety and accessibility.
- Assess muscle strength, mobility, gait, and range of motion.
- Direct physical therapy treatment and instruct patients and caregivers in the use and care of therapeutic appliances.
- Train patients in the use of prosthetic devices.
- Prepare and submit clinical notes and progress summaries based on goal attainment.
- Participate in discharge planning and interdisciplinary case conferences.
- Provide physical therapy consultation to families/caregivers and in-service education programs for staff.
- Supervise Physical Therapy Assistants in accordance with policy and state regulations.
