Physical Therapist ( PT) PRN
About The Position
Cypress Ridge Care Center is seeking a skilled and compassionate Physical Therapist to join their therapy team on a PRN or Part-Time basis. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide high-quality therapeutic care to support mobility, function, and overall quality of life. Requirements
- Current Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Strong communication and documentation skills
- Ability to work independently and as part of a team
Nice To Haves
- Experience in a skilled nursing or long-term care setting preferred
Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans
- Provide direct physical therapy services in accordance with physician orders
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Collaborate with interdisciplinary team members to ensure coordinated resident care
- Educate residents and caregivers on treatment plans, exercises, and safety techniques
- Maintain compliance with facility policies, state regulations, and professional standards
