About The Position

Anew Home Health is seeking a dedicated Physical Therapist for PRN hours in the Mooresville / Martinsville/ Greenwood / Camby, IN area. The role involves providing physical therapy services according to a physician's plan of care and state Physical Therapy Practice Act, implementing quality services with minimal supervision, evaluating clients, developing treatment and discharge plans, contributing to case management, and adhering to state regulations. The position also involves referring/supervising patients for treatment to a physical therapist assistant (PTA).

Responsibilities

Provides physical therapy services to patients per a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
Implements, with minimal supervision, quality physical therapy services for patients in accordance with the principles and practices of physical therapy and within agency policies and procedures.
Evaluates clients, develops initial and ongoing treatment plans, develops discharge plans, contributes to case management and provides quality patient care.
Adheres to applicable state regulations concerning physical therapy and refers/supervises patients for treatment to physical therapist assistant (PTA).
