Requirements
- Kentucky license as a physical therapist required.
- Requires a Bachelor, Master or Doctoral degree from a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE), or higher.
- BLS preferred at hire, required within 90 days after hire.
