About The Position

CareCo Medical Home Health is seeking a licensed Physical Therapist to provide skilled in-home therapy services to patients in the community. This is an excellent opportunity for an experienced clinician who enjoys working independently while collaborating with a supportive interdisciplinary team. Our services include nursing, therapy, and social services, all designed to help individuals remain safe, strong, and independent in their homes. We manage acute and chronic conditions, provide wound care, medication management, and comprehensive therapy services. Position Summary The Physical Therapist plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role may also include case management for patients requiring skilled therapy services only. Therapists work collaboratively with nurses and other disciplines to provide care, education, and support to patients and their caregivers under the supervision of the Clinical Supervisor.

Requirements

  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist license in the state of practice (Connecticut)
  • Minimum of three years of healthcare experience
  • Strong communication and documentation skills
  • Ability to work independently with minimal supervision
  • Valid drivers license and reliable, insured vehicle

Nice To Haves

  • Home health and OASIS experience preferred

Responsibilities

  • Perform physical therapy evaluations, assessments, and skilled treatments in the home
  • Complete initial and comprehensive assessments, including OASIS, within required timeframes
  • Develop, implement, and update individualized Plans of Care
  • Document all visits, findings, and outcomes in a timely manner
  • Communicate changes in patient status to physicians, case managers, and the care team
  • Provide patient and caregiver education to promote recovery and safety
  • Participate in discharge planning and interdisciplinary care coordination
  • Supervise Physical Therapy Assistants in accordance with agency and regulatory guidelines
  • Promote patient safety and comply with all agency, state, and federal standards
