About The Position
We are seeking a dedicated and compassionate Physical Therapist to join our home health team serving patients in Suisun City. This is a flexible, part-time opportunity offering approximately 2–3 visits per week, ideal for a clinician who values autonomy and delivering high-quality, patient-centered care in the home setting.
Requirements
- Active Physical Therapist license in the State of California
- Strong clinical assessment and patient care skills
- Ability to work independently and manage a flexible schedule
- Excellent communication and interpersonal skills
Nice To Haves
- Prior home health experience preferred, but not required
Responsibilities
- Provide skilled physical therapy services in patients’ homes in accordance with the plan of care
- Evaluate patient needs and develop individualized treatment plans
- Implement therapeutic interventions to improve mobility, reduce pain, and restore function
- Monitor patient progress and adjust care plans as appropriate
- Educate patients and caregivers on treatment plans, exercises, and safety measures
- Maintain timely and accurate documentation
- Collaborate with physicians and interdisciplinary team members to ensure continuity of care
