As a Physical Therapist at Haxtun Health, you'll be a vital part of our team, acting as a "movement detective" to evaluate and pinpoint the root causes of patients' pain and limitations. You will develop personalized rehabilitation programs to enhance mobility, alleviate pain, and restore function, directly witnessing the positive impact of your hands-on expertise. Beyond clinical care, you'll have opportunities to nurture community wellness through local outreach and preventative programs while building strong, supportive relationships with both patients and colleagues. Located on the beautiful plains of northeast Colorado, Haxtun Health is a 25-bed Critical Access Hospital and Primary Care Clinics, including long-term care, rehabilitation, and specialty care. Our mission is to promote optimum wellness through compassionate and high-quality care. Employing nearly 150 people, Haxtun Health is one of the largest employers in the community. We recently opened our new 100,000 square foot facility that houses our Family Practice Clinic, Wellness and Rehabilitation, and Retail Pharmacy. Requirements

  • Graduate of an accredited program in physical therapy (MSPT or DPT)
  • Current license as a Physical Therapist in the State of Colorado
  • Basic Life Support (BLS)

Nice To Haves

  • 3 (three) years’ experience in inpatient and outpatient setting
  • Trigger Point Dry Needling Certification

  • Paid Time Off (PTO): Accrued at approximately 17 days for the first year of employment for hourly employees and 22 days for the first year of employment for salaried employees. PTO accrual increases as your length of service increases. This includes holidays.

  • Retirement Plan: Up to a 3% employer match, which increases with your length of service. New employees are automatically enrolled at a 1% contribution rate (can be opted out). Employees must complete 5 years of employment to be 100% vested.
  • Employee Health Payment Match: Haxtun Health matches employee payments up to $1,000 per year per family for services received at Haxtun Health’s hospital or clinics.
  • Family and Medical Leave Insurance Program (FAMLI): Haxtun Health participates in the CO FAMLI program, which is a social insurance program that provides up to 12 weeks of paid leave for qualifying life events.
  • Term Life Insurance: 1 x annual salary term life provided by HHD, no employee premium ($200,000 Max.). Additional voluntary life insurance coverage for the employee and family is an option which can be purchased by the employee.
