Physical Therapist (Per Diem) - PACE
About The Position
This is an on-call/per diem position supporting PACE sites in Seattle and Kent, WA. The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA. Providence PACE is a Program of All-Inclusive Care for the Elderly that strives to keep older adults as healthy as possible living in the community through clinics, home visits and more. Requirements
- Bachelor's Degree in Physical Therapy
- National Provider BLS - American Heart Association within 30 days of hire
- Washington Physical Therapist License upon hire
Nice To Haves
- Master's Degree in Physical Therapy or
- D.P.T. in Physical Therapy
- 1 year working with a geriatric population
