About the Role: We are seeking a Physical Therapist (PT) to provide in-home, individualized therapy to adult clients in Pueblo, CO. You’ll work directly with patients to improve mobility, function, and quality of life through personalized care. Compensation & Scheduling: Full-Time and Part-Time Schedules Competitive Pay: $70.00 – $85.00 per routine visit Flexible Schedule: Designed to support a healthy work-life balance while meeting your professional goals
Requirements
- Licensed Physical Therapist in Colorado
- Experience in home health or a related field
- Strong communication and interpersonal skills
- Proficient in electronic medical record documentation
- Ability to develop and implement individualized treatment plans
- Flexibility to adapt to diverse client needs
Responsibilities
- Develop and implement individualized treatment plans
- Conduct assessments using standardized tools and clinical judgment
- Provide therapy focused on mobility, strength, balance, and safety
- Educate patients and caregivers on home exercise programs and adaptive strategies
- Maintain accurate and timely clinical documentation
- Collaborate with a multidisciplinary care team to ensure coordinated patient care
