Requirements

Licensed Physical Therapist (PT) in the state of New Mexico
At least 1 year of clinical experience (home health preferred)
Skilled in patient communication, documentation, and problem-solving
Self-motivated and comfortable working independently
Proficient with electronic health records and mobile charting
Passionate about function-focused, relationship-driven care

Responsibilities

Evaluate patients and create personalized therapy plans in the home setting
Deliver evidence-based interventions to restore mobility and function
Educate patients and families on exercises and safety strategies
Collaborate with the interdisciplinary team to ensure coordinated, goal-oriented care
Maintain accurate and timely documentation to meet quality and compliance standards
Adapt care plans based on progress and physician feedback
Support patients through discharge planning and wellness strategies
