About The Position

Physical Therapist (PT) – Home Health Pembroke Pines Part-Time Opportunities Available Paid per visit – routine visit rates starting at $75 per visit! Nightingale Home Healthcare– Compassionate Care. We’re seeking a skilled Physical Therapist (PT) to join our team and help patients regain independence through one-on-one home health care. Requirements

  • Active Physical Therapist (PT) license in FL
  • Physical exam within the last 12 months
  • Current TB test (within 12 months) and CPR certification
  • Valid driver’s license and reliable transportation

Nice To Haves

  • Minimum 1 year of PT experience preferred

Responsibilities

  • Provide effective patient care using approved therapy techniques and procedures
  • Evaluate patient condition and establish measurable treatment goals
  • Develop and implement individualized treatment plans
  • Supervise and coordinate care with PTAs and HHAs
  • Document patient progress, updates, and treatment notes accurately and on time
  • Educate patients and families on exercises, mobility, and use of adaptive equipment
  • Evaluate home environments and recommend modifications to improve safety and independence
  • Ensure all services meet HIPAA standards for confidentiality
  • Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
