Requirements
- Licensed Physical Therapist (PT) in good standing
- Strong clinical skills with a patient-first mindset
- Excellent communication and documentation abilities
- Ability to work independently while contributing to a collaborative team
- Commitment to quality care and positive patient outcomes
- Successful completion of accredited physical therapy program and state licensure exam.
- Valid Physical Therapy license
- CPR certification
- Valid Driver’s license
- Valid Auto Insurance
Nice To Haves
- 1 year of home health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Deliver hands-on, goal-driven physical therapy to improve mobility, strength, and function
- Track patient progress and adjust care plans to maximize outcomes
- Educate patients and families to support recovery and long-term success
- Maintain timely and accurate clinical documentation
- Collaborate with healthcare team members to ensure coordinated, high-quality care
