The Physical Therapist evaluates, plans, and administers treatments for disabilities, injuries, and diseases in accordance with a physician's order. This role is responsible for coordinating care to meet identified Physical Therapy needs of the patient/family, participating in patient care management including identification, monitoring care delivery, advocating for the patient, evaluating outcomes, facilitating patient care across the health care continuum, and assisting other team members. The position also serves as a preceptor for related staff and students. For Physical Therapists working with the Pediatric Population, responsibilities include providing pediatric assessment, treatment, and family/staff consultation and education services, developing individualized treatment plans, actively involving families in goal setting and discharge planning, participating in multidisciplinary pediatric team meetings, providing education and consultation for pediatric staff development, and collaborating with other providers like CCS case managers, Local Education Agency, Regional Center, or other pediatric programs to ensure continuity of care.

Requirements

  • California State Physical Therapist License
  • Graduate of an accredited Physical Therapy Program
  • Current Physical Therapy License issued by Physical Therapy Board of California
  • Current Basic Life Support Provider (BLS) issued by American Heart Association

Responsibilities

  • Evaluates, plans and administers treatments for disabilities, injuries and diseases in accordance with a physician's order
  • Responsible for coordination of care to meet identified Physical Therapy needs of the patient/family
  • Participates in the management of patient care to include identification, monitoring care delivery, advocating for the patient, evaluating outcomes, facilitating patient care across the health care continuum, and assists other team members with meeting patient needs
  • Is preceptor for related staff and students
  • Provides pediatric assessment, treatment, and family/staff consultation and education services (for Physical Therapists working with Pediatric Population)
  • Develops a treatment plan from evaluation findings (for Physical Therapists working with Pediatric Population)
  • Provides age specific Physical therapy intervention as defined by the individualized treatment plan (for Physical Therapists working with Pediatric Population)
  • Actively involves the family and/or primary care giver when establishing goals from initial assessment, through the course of treatment and in discharge planning (for Physical Therapists working with Pediatric Population)
  • Participates in team meetings regarding pediatric patients who are receiving Physical therapy intervention (for Physical Therapists working with Pediatric Population)
  • Provides education and consultation for pediatric staff development regarding PT related topics pertinent to overall patient care, in collaboration with the pediatric department clinical nurse specialist and/or nurse educator (for Physical Therapists working with Pediatric Population)
  • Collaborates with other providers to facilitate the continuum of care (e.g., coordinating with a CCS case manager and/or a representative of the CCS Medical Therapy Program for referral to a CCS-paneled provider or CCS MTU for the patient with a CCS-eligible condition who may continue to require physical therapy services after hospital discharge) (for Physical Therapists working with Pediatric Population)
  • Communicates with other health, education and social service providers such as the Local Education Agency, Regional Center or pediatric inpatient/outpatient programs to assure continuity of intervention (for Physical Therapists working with Pediatric Population)
